As a friendly reminder, all patients must fill out paperwork prior to EVERY visit including follow-up appointments so we are prepared for your arrival.
Also, NO dental work (including dental cleanings) or vaccines two weeks before or two weeks after filler. Lastly, please tell us if you are taking an antibiotic or steroid when scheduling an appointment to be sure these medications do not interfere with your planned treatment with Dr. Polacek.
